These two forms are for every event or trip that requires students to turn-in their medications. This keeps them safe, you safe, and your group safe. These forms are made so you can customize them if you choose to use different language. Simply have parents fill out the form, provide zip-lock baggies for parents to place medications into, and assign a sponsor of staff member to be in charge of distribution.

CAUTION: Before distributing these forms, have your church administration review, edit as necessary, and okay their use.
